Neue Ansätze zur Psychodynamik phobischer Symptombildungen zentrieren auf die Bedeutung des psychischen Raums bzw. psychischer Dreidimensionalität. Ausgehend von Freuds energetischer und psychologischer Angsttheorie, von M. Kleins Unterscheidung von Verfolgungsangst und depressiver Angst sowie von Bions Erweiterung der Metapsychologie um einen geographischen Gesichtspunkt, werden die verschiedenen Angstformen unter dem Aspekt des Erhalts bzw. der Einschränkung psychischen Raums betrachtet. Dies erlaubt eine geographische Hierarchie von Angstformen zu formulieren, die für die Erfassung von Ängsten in der Übertragungsbeziehung bedeutsam ist. Abschließend werden behandlungstechnische Aspekte, insbesondere die angstmindernde Funktion des Deutens, untersucht.

Drive, object, space. Changes in the psychoanalytic understanding of anxiety
New approaches to the psychodynamics of phobic symptom-formations center on the meaning of psychic space and/or psychic three-dimensionality. The article takes its point of departure from Freud's energistic and psychological theory of anxiety, M. Klein's distinction between persecution anxiety and depressive anxiety, and Bion's extension of metapsychology to include a geographical aspect, drawing on these to discuss various forms of anxiety from the point of view of the preservation or restriction of psychic space. This permits the framing of a geographical hierarchy of anxiety forms that is of significance for the recognition and definition of anxiety in the transference relationship. The article closes with a discussion of various technical aspects of therapy, notably the function of interpretation in the reduction of anxiety.

Pulsion, objet, espace. Les transformations de la conception psychanalytique de l'angoisse
Les récentes approches concernant la psychodynamique des formations du symptôme phobique mettent au centre la signification de l'espace psychique, c'est-à-dire le caractère tridimensionnel de celui-ci. En partant à la fois de la théorie énergétique et psychologique de l'angoisse selon Freud, et de la distinction, opérée par M. Klein, entre l'angoisse de persécution et l'angoisse dépressive, et de l'élargissement de la métapsychologie, opérée par Bion, à un point de vue géographique, l'auteur analyse les différentes formes d'angoisse sous l'angle de la conservation ou de la restriction de l'espace psychique. Cela permet d'établir une hiérarchie géographique des formes d'angoisse, importante pour la compréhension des angoisses dans la relation de transfert. Pour conclure, l'auteur aborde des aspects de la technique thérapeutique, notamment la fonction interprétative qui réduit l'angoisse.

In einer dichten Beschreibung (Clifford Geertz) der Krankengeschichte des Kleinen Hans erörtert der Autor die These, daß die Pferdephobie nicht, wie bei Freud, auf den Erwerb des Kastrationskomplexes verweist, sondern Symptom einer traumatischen Verarbeitung der Beschneidung ist. Zur Begründung seiner These verweist Maciejewski auf den Zusammenhang der Beschneidung und einer Tonsillektomie des Kleinen Hans, die das Beschneidungstrauma reaktivierte und zum Ausbruch der Phobie führte. Einer weiteren These des Autors zufolge ist die abgeschnittene Vorhaut als Symbol der verlorenen eigenen Weiblichkeit des beschnittenen Knaben zu sehen, die in der Vulva wiedererkannt wird. Für die zukünftige Forschung, so der Autor, ergibt sich aus dieser neuen Sichtweise die Aufgabe einer Neubewertung des Verhältnisses von Trieb- und Traumatheorie, bei der auch der Mutterkomplex an Bedeutung gewinnen wird.

A »thick description« of Little Hans. On the forgotten trauma of circumcision
In a thick description (Clifford Geertz) of Little Hans' case history, the author expounds his thesis that the boy's fear of horses is not (as in Freud) an indication of the acquisition of the castration complex but the symptom of a traumatic engagement with circumcision. To substantiate his theory, Maciejewski points to the connection between circumcision and a tonsillectomy performed on Little Hans, which reactivated the trauma and led to the outbreak of the phobia. In addition, the author argues that the removal of the foreskin should be seen as a symbol of the lost femininity of the circumcised boy, re-identified in the vulva. The author suggests that for future research this new perspective implies the task of reevaluating the relationship between drive theory and trauma theory, a process in which the mother complex will also take on greater significance.

A propos d'une »description dense« du petit Hans. Sur le traumatisme oublié de la circoncision
Dans sa »description dense« (Clifford Geertz) de l'histoire de la maladie du petit Hans, l'auteur discute la thèse selon laquelle la phobie des chevaux ne renvoie pas, comme le dit Freud, à l'acquisition du complexe de castration, mais est le symptôme d'une élaboration traumatique de la circoncision. Pour fonder sa thèse, Maciejewski indique le rapport qui existe entre la circoncision et une tonsillectomie ultérieure du petit Hans, celle-ci ayant réactivé le traumatisme de la circoncision et déclenché la phobie. Selon une autre thèse de l'auteur, le prépuce coupé doit être considéré comme symbole de la féminité perdue du garçon circonci, et il est reconnu dans la vulve. D'après l'auteur, cette conception implique, pour la recherche, la tâche de réévaluer le rapport entre la théorie des pulsions et la théorie du trauma, réévaluation qui augmentera aussi l'importance du complexe maternel.
