Der Autor diskutiert den seit einigen Jahren schwelenden Konflikt zwischen denen, die krankenkassenfinanzierte psychoanalytische Behandlungen befürworten, und denen, die diese Regelung mit der Begründung ablehnen, sie sei mit den Heilungszielen der Psychoanalyse unvereinbar. Dabei kommt er zu dem Schluß, daß die »reine« Analyse, von der Freud sprach, eine normative Vorstellung ist, die von der Realität psychoanalytischer Behandlungen permanent dementiert wird: Die stete Präsenz des Dritten in der Analyse – die Präsenz der Gesellschaft und sozialer Wertvorstellungen bei Analytiker und Patient – ist ein Faktum, das die Frontstellung Eigenfinanzierung-Fremdfinanzierung als der Sache unangemessen erscheinen läßt. Vielmehr plädiert der Autor dafür, die Problematik auf technischer Ebene anzugehen, weil sie nur hier sachlich und mit Aussicht auf Lösungen zu diskutieren sei. Ohne die Widersprüche und Schwierigkeiten, die sich aus der Kassenfinanzierung ergeben, zu vernachlässigen, kommt der Autor zu dem Resultat, daß die Psychoanalyse allein um den Preis gesellschaftlicher Isolation und therapeutischer Stagnation – was am Beispiel der Situation der Psychoanalyse in den USA gezeigt wird – auf Fremdfinanzierung verzichten kann.

The presence of the third party in psychoanalysis: The problern of payment by others
For several years a conflict has been smouldering between those who favor psychoanalytic treatment financed by medical insurance plans and those who oppose this arrangement on the ground that it is incompatible with the therapeutic goals of psychoanalysis. In discussing this conflict, the author comes to the conclusion that »pure« analysis, as conceived by Freud, is a normative concept which is continually negated by the reality of psychoanalytic treatment. The steady presence of third parties in analysis – the presence of society and of social norms for analyst and patient – is a fact in the light of which the confrontation around third party payments seems disproportionate. Instead the author urges that this problem be approached on the technical level. Only in this way can it be addressed realistically and with a hope for solutions. Without attempting to ignore the difficulties and contradictions arising from insurance financing, the author concludes that psychoanalysis can dispense with third party financing only at the price of social isolation and therapeutic stagnation, citing the situation of psychoanalysis in the USA as an example.

Vorgestellt wird der familientherapeutische Ansatz zum Verständnis und zur Behandlung von Schizophrenie. Dem Therapeuten kommt in diesem Rahmen vor allem die Aufgabe zu, den »malignen Clinch«, zu dem die Interaktionen der Familienangehörigen entartet sind, aufzubrechen, d.h. die seither indiskutablen Familiengeheimnisse, Delegationen, Loyalitäten, Kränkungen und Ausbeutungssituationen zur Sprache zu bringen, um auf diesem Wege die Beziehungsrealität der Familie tu reformieren.

The »relational« reality of schizophrenics
The author presents family therapy as an approach to the study and treatment of schizophrenia. It is the therapist's primary task to break up the »maligne clinch« to which the interactions of the family members have degenerated, that is, to verbalize family secrets, delegations, loyalties, mortifications, and exploitative situations which had been barred from communication. In this way the relational reality of the family is reformed.

Ciompi plädiert für eine Synthese von psychoanalytischer Theorie und allgemeiner Systemtheorie: eine »psychoanalytische Systemtheorie«. Deren Vorteile werden im Hinblick auf die Trieb-, Ich- und Narzißmus-Theorie, besonders aber am Beispiel der Familiendynamik aufgezeigt.

Ciompi pleads for a synthesis of psychoanalytic theory and general systems theory: a »psychoanalytic systems theory«. The advantages of such a synthesis are demonstrated in regard to the study of drives, the ego, narcissism, and especially family dynamics.
