Die deutschen Psychotherapie-Richtlinien beschreiben die tiefenpsychologisch fundierte Psychotherapie als Minus-Variante der analytischen Therapie hinsichtlich Therapiedauer, Sitzungsfrequenz, Nutzung der Übertragung und therapeutischer Regression. Obwohl sie weitaus häufiger angewendet wird als die analytische Psychotherapie, fehlt bisher eine umfassende Theorie ihrer therapeutischen Wirkungsweise. Klinische Erfahrungen legen nahe, dass mindestens drei Elemente zu ihrer Wirksamkeit beitragen: (1) ein erweitertes psychodynamisches Beziehungsverständnis, das nicht auf die Arbeit an der Übertragungsbeziehung eingeschränkt ist, (2) das Prinzip der Ressourcenaktivierung und (3) die Bereitschaft zur reflektierten Integration von Interventionen aus anderen Therapieschulen.

Psychodynamic psychotherapy as a resourcebased integrative method
German Psychotherapy Guidelines describe psychodynamic psychotherapy as a minor variant of analytical psychotherapy with respect to therapy duration, session frequency, use of transference, and therapeutic regression. Even though it is far more extensively used compared to analytical therapy, a comprehensive theory of its therapeutic action is still lacking. Evidence from clinical practice suggests that at least three elements contribute to its therapeutic action: (1) an broadened psychodynamic understanding of relationships which is not restricted to transference work, (2) the principle of resource activation, and (3) a tendency to reflectively integrate therapeutic interventions from other therapy schools.

Ausgehend von der therapeutischen Erfahrung bei der Behandlung narzisstischer Persönlichkeitsstörungen wird deren Verletzlichkeit und Grandiosität mit den offenkundigen Bindungsschwierigkeiten in Verbindung gebracht. Beginnend mit Freuds Narzissmusbegriff und der heute noch aktuellen Differenzierung des Narzissmusverständnisses bei Kohut und Kernberg wird die dichotome Erfahrung des Narzissmus in seiner vulnerablen und grandiosen Form in Verbindung mit spezifischen Bindungsmustern und Mentalisierungsstrategien gebracht. Über die Polarität eines anaklitischen und introjektiven Modus werden entwicklungspsychologische Ergänzungen eingebracht, die ein integratives Verständnis des Hintergrunds narzisstischer Symptomatik ermöglichen.

Narcissism between fear of attachment and wish for attachment
From the point of view of therapeutic experience during treatment of narcissistic personality disorders vulnerability and grandiosity is connected with evident difficulties concerning attachment. Starting with Freuds conception of narcissism the different understanding in Kohut's and Kernberg's theories including the dichotomy of vulnerable and grandiose narcissism in connection with specific attachment patterns and strategies of mentalization will be explained. The polarity between anaclitic and introjective mode will be completed by developmental psychological explanations enabling the reader for an integrative understanding of the background of narcissistic symptomatology.

Psychotherapie scheint sich zunehmend aus dem Therapiezimmer in den virtuellen Raum zu bewegen. Studien belegen die Wirksamkeit der neuen Kommunikations- und Behandlungsformen. Forscher und Krankenkassenvertreter feiern die verheißenen Kostenersparnisse, die Reduzierung von Wartezeiten sowie die Niederschwelligkeit des Zugangs. Zugleich gibt es Bedenken: Viele Therapeuten befürchten, arbeitslos oder zumindest in Honorierung und Bedeutung beschnitten zu werden. Insbesondere die psychodynamischen Psychotherapeuten beklagen den Verlust der personalen Begegnung und betonen die Bedeutung der therapeutischen Beziehung. Dem Trend in der Therapielandschaft folgend handelt es sich bei den internetbasierten Verfahren überwiegend um manualisierte behavioral-kognitive Konzeptionen. Im folgenden Beitrag werden die neueren Entwicklungen von Beratungs- und Therapieformen im Internet vorgestellt und problematisiert.

(Psychodynamic) therapy in the internet age
Psychotherapy seems to move increasingly from the therapy room into virtual space. Studies confirm the effectiveness of the new communication forms and forms of treatment. Researchers and health insurance scheme representatives celebrate the cost savings, the reduction of waiting periods as well as the low threshold of the access. At the same time there is doubt: Many therapists fear to lose their job or at least a reduced income and importance. In particular psychodynamic psychotherapists complain about the loss of the personal meeting and stress the meaning of the therapeutic relationship. With respect to the trend in the therapy scenery the Internet-based procedures seem predominantly to be manualised behavioural cognitive concepts . In the following article the newer developments are introduced by consultation and therapy forms on the Internet discussed.

Psychoanalytisch begründete Therapieverfahren finden breite Anwendung in der Richtlinien-Psychotherapie. Ein besonderer Stellenwert kommt dabei der tiefenpsychologisch fundierten Psychotherapie zu. Es gibt nur wenige empirische Untersuchungen zur Differenzialindikation zwischen Analytischer Psychotherapie (AP) und Tiefenpsychologisch fundierten Therapieverfahren (TFP), speziell unter Berücksichtigung des Versorgungsalltags und der Rahmenbedingungen in Deutschland, wie sie mit der Richtlinien-Psychotherapie (RLPT) gegeben sind. Auf der Grundlage von n=400 zufällig ausgewählten Patienten in der Richtlinien-Psychotherapie (psychoanalytisch begründete Verfahren) wird im Rahmen des MARS-Projektes nach differenzialindikatorischen Kriterien Ausschau gehalten, wie sie unter Praxisbedingungen realisiert werden. Wie sich zeigt, kommt klinischen Variablen dabei keine gesicherte Bedeutung zu. Stattdessen schlagen eher soziale Faktoren durch. Weitere Forschungen werden für notwendig gehalten, um eine gesicherte und an der Klinik orientierte Vorgehensweise zu gewährleisten.

Depth-psychological orientated and analytic psychotherapy
Psychoanalytic therapies are widely used in the German guideline psychotherapy. A particular emphasis is placed on psychodynamic psychotherapies or some kind of depth-psychological orientated psychotherapy (TFP). Despite the decades of experience and fundamental conceptualizations, a demarcation of these different methods and an empirical foundation in everyday- and research practice is difficult to implement. There are few empirical studies on the differential indication between analytic psychotherapy (AP) and TFP, especially taking into account the everyday health care and the environment in Germany, as they are given in the guideline psychotherapy. Of main interest of the study, based on n=400 randomly selected patients in guideline-psychotherapy (psychoanalytic founded psychotherapies, AP and TFP), is the outlook on the practice in differential indication under daily providing conditions. The results suggest, that clinical variables do not possess a secure importance on the decision between AP and TFP. Thus the results confirm the important role of the variables gender, degree of education of the patient as well as the regional location of treatment (rural or urban) on the therapeutical proceeding. These results are discussed.

Das im Teil 1 (PDP 2014; 13 [3]: 162-175) vorgestellte entwicklungspsychopathologische Modell des pathologischen PC/Inernetgebrauchs umfasst das Gaming und Chatting als die aktuell häufigsten Varianten dieses neuen Störungsbildes. Die frühe Entstehung und Starre des Verhaltensmusters mit der hervorstechenden sozialen Beziehungsstörung legt die nosologische Einordnung als Persönlichkeits- und Verhaltensstörung nahe. Mit dem Kurzfragebogen zu Problemen beim Computergebrauch (KPC) liegt ein klinisch validiertes, mehrdimensionales Screening-Verfahren vor. Die Befunde zu neurobiologischen Korrelaten des pathologischen PC/Internetgebrauchs sind noch bruchstückhaft und nicht geeignet, das Phänomen zu begreifen. Die Befunde zu möglichen Risikofaktoren der Persönlichkeit verweisen konsistent auf eine erhöhte Depressivität und (soziale) Ängstlichkeit sowohl in Persönlichkeitstests als auch bei der Erfassung komorbider Störungen. Als störungsspezifisches Merkmal hat sich bisher lediglich die erniedrigte Gewissenhaftigkeit, d.h. die verminderte Fähigkeit, im Alltag mit Ausdauer Probleme zu lösen, erwiesen. Für die Effektivität der Behandlung des pathologischen PC/Internetgebrauchs liegen international vorläufige metaanalytische Befunde vor. In Deutschland konnten für die stationäre Behandlung katamnestisch ähnlich hohe Erfolgsquoten wie bei der Behandlung des pathologischen Glücksspielens und der Alkohol- und Medikamentenabhängigkeit belegt werden.

Pathological PC/internet use: disorder, treatment and research, part 2
The in part 1 (PDP 2014; 13 [3]: 162-175) introduced developmental psychopathology model of pathological PC/internet use includes Gaming and Chatting as the most frequent variations of this new disorder. The early origin and rigidity of the behaviour pattern with the disturbance of social relationships suggests the nosologic classification as a personality disorder and behavioural disorder. With the short questionnaire to problems with the computer use (KPC) a clinically validated, multidimensional screening procedure is given. The findings to neuro-biological correlates of the pathological PC/internet use are still fragmentary and not to understand the phenomenon. The findings to possible risk factors of the personality refer consistent to a raised depressiveness and (social) anxiety in personality tests as well as with the capture of comorbid disorders. Merely the degraded conscientiousness has turned out to be specific for this disorder, i.e. the decreased ability to solve problems in the everyday life with perseverance. For the effectiveness of the treatment of the pathological PC/internet use temporary metaanalytic findings are given internationally. In Germany for the inpatient treatment similarly high success rates are attained like with the treatment of pathological gambling and alcohol and medicine depencency could be verified.
